Trigger points refer pain away from themselves, which is why the spot that hurts often isn't the spot that needs treating. These are the patterns we see most.
Upper Trapezius

How we find the right spot
An injection into the wrong muscle does nothing. Most of the value in this procedure is in the five minutes of examination before the needle comes out:
- Mapping where the pain goes. We ask you to trace it. A referral pattern that runs from the shoulder to behind the eye points to a very different muscle than one that stays put.
- Palpation for the taut band. Trigger points feel like a firm cord within the muscle with an exquisitely tender nodule along it — findable by hand, not on a scan.
- Reproducing your symptom. Pressing the right point recreates the pain you actually came in with. That confirmation is what makes it the right target.
- The twitch response. A brief involuntary contraction when the band is engaged — the clearest sign the needle is where it should be.
- Ruling out what isn't muscular. Radiculopathy, a rotator cuff tear, inflammatory arthritis, and referred pain from an organ all mimic myofascial pain. Being a full medical practice means we can order imaging and labs and act on them rather than injecting and hoping.
- Asking why it keeps happening. Workstation setup, an old injury, unrefreshing sleep, or an untreated chronic condition. Injecting the same knot every six weeks without answering that question isn't treatment, it's maintenance.

Does the injection hurt?
Most patients describe the injection as a brief pinch or pressure sensation, lasting only seconds. Many people feel immediate relief that far outweighs the momentary discomfort.
How quickly will I feel relief?

What's actually in the injection?
Typically a small amount of local anesthetic, which numbs the area immediately and lets the muscle release. We don't use corticosteroids for trigger point injections — when extra anti-inflammatory support is needed, we use Traumeel, a homeopathic formula for pain and inflammation backed by clinical research on its effectiveness, instead. In some cases the needle alone is used with no medication at all — the mechanical disruption of the taut band is doing the work. We tell you which of these we're using and why before we start.
Are there side effects, or reasons not to have one?
The common effects are minor: soreness at the site for a day or two, occasional bruising, and rarely a brief lightheaded feeling right afterward. We avoid injecting through infected or broken skin, and we take extra care with patients on blood thinners, with bleeding disorders, or with a known local-anesthetic allergy — which is why your full medication list gets reviewed first.
