What Is Spravato Treatment?

Spravato (esketamine) is an FDA-approved nasal spray for adults with treatment-resistant depression who haven't found relief from other antidepressants. If you've tried two or more medications without success, Spravato offers a new path forward.

Unlike traditional antidepressants that can take weeks to work, many patients experience improvement within hours or days. Spravato works differently than other depression medications by targeting NMDA receptors in the brain, helping restore neural connections that depression has weakened.

Treatment takes place in our comfortable, private treatment spaces. Each session lasts about 2 hours including monitoring time. You'll self-administer the nasal spray under our supervision, then rest while we monitor your response.

What to Expect at Your Spravato Appointment

  1. Initial Consultation (30–45 minutes)

    We review your depression history and previous treatments to determine if Spravato is right for you, and handle insurance verification and pre-authorization. Available in person or by telemedicine.

  2. Treatment Session (about 2 hours)

    You'll arrive at our comfortable treatment room and self-administer the nasal spray under our supervision, then rest while we monitor your response.

  3. Monitored Rest Period

    You'll rest in our private treatment space while we track your vital signs and response. Many patients describe feeling relaxed; you can listen to music or simply rest.

  4. Going Home

    Once your provider confirms you're stable, you can go home — you'll need someone to drive you. Most patients return to normal activities the same day or next morning.

  5. Ongoing Reassessment

    Initially twice a week, then once weekly, then monthly for maintenance — with regular check-ins on your progress, not just a fixed schedule regardless of how you're responding.

  • Bring someone to drive you home — you cannot drive for 24 hours after treatment
  • Avoid eating for 2 hours before your appointment

We reassess your symptoms at regular intervals throughout your treatment course, not just at intake, adjusting your schedule and coordinating with any therapy or medication management you're also receiving.

Why do I have to stay for two hours and have someone drive me?

Because it's federally required, not because we're being cautious for the sake of it. Spravato is only available through a restricted program (a REMS) that requires it be self-administered at a certified treatment center and that you're monitored on site for at least two hours afterward, with blood pressure checked along the way. You can't drive for the rest of the day. It's the same reason Spravato isn't a prescription you can pick up at a pharmacy and use at home — and it's exactly what makes a practice with 30,000+ treatments behind it worth choosing.

What does it actually feel like, and what are the side effects?

Most patients describe a floaty, detached, slightly dreamlike feeling during the session that fades well before they go home. The common side effects are dissociation, dizziness, nausea, a temporary rise in blood pressure, sedation, and an odd taste — and they generally get milder over a course of treatment as your body adapts. We monitor for all of it in the room. If nausea is an issue for you, that's manageable, and it's part of why we ask you not to eat for two hours beforehand.

How soon might I notice a difference?

Faster than a conventional antidepressant, which is much of the point — some patients notice a shift within the first week or two, where an SSRI typically needs four to six weeks. That said, it isn't universal and it isn't instant for everyone. Treatment starts at twice weekly for the first month, then moves to weekly, then to a maintenance interval based on how you're actually doing. We reassess your symptoms throughout rather than running you through a fixed schedule and hoping.
