Use of this website and communication with MyPremierMD through this website does not establish a doctor-patient relationship. A doctor-patient relationship is only established when you become a patient at one of our offices and receive care from our providers.

Emergency Situations.

If you are experiencing a medical emergency, call 911 immediately. Do not use this website to communicate urgent medical needs.

3. User Accounts and Registration

To access certain features of our Service, including our patient portal and online appointment scheduling, you may be required to create an account. When creating an account, you agree to provide accurate, current, and complete information; maintain the security of your password; and take responsibility for all activities that occur under your account. You may not share your account credentials with anyone else.

4. Privacy and Protected Health Information

Our collection, use, and disclosure of your personal information and protected health information (PHI) is governed by our Privacy Policy and HIPAA. We use industry-standard security measures to protect your information, but no method of transmission over the internet is 100% secure.

5. Appointment Scheduling and Cancellations

Cancellation Policy: if you need to cancel or reschedule an appointment, please notify us at least 24 hours in advance when possible.

No-Show Policy: repeated missed appointments without notice may result in limitations on future scheduling or discharge from the practice.

Same-Day Appointments: while we strive to accommodate same-day appointments, availability cannot be guaranteed.

6. Telemedicine Services

MyPremierMD offers telemedicine services for eligible patients and conditions. By using our telemedicine services, you acknowledge that telemedicine has limitations and may not be appropriate for all conditions; you are responsible for a private, secure location and a reliable internet connection; and your provider may determine an in-person visit is necessary. The same privacy and confidentiality standards apply as in-person visits. If you experience technical issues during a telemedicine appointment, call our office immediately at (908) 450-7002.

7. Payment and Insurance

You are responsible for payment of all services rendered, due at the time of service unless other arrangements have been made in advance. We accept most major insurance plans, but cannot guarantee coverage for any specific service — you are responsible for understanding your insurance benefits. If your insurance does not cover a service, you will be responsible for the full cost.

8. Prohibited Uses

You agree not to use this website or Service to violate any law; transmit harmful, threatening, or defamatory content; impersonate any person or entity; interfere with or disrupt the Service; attempt unauthorized access; collect data about other users without permission; or upload viruses or malicious code. We reserve the right to terminate access for any violation.

Any legal action arising out of these Terms shall be brought exclusively in the federal or state courts located in Somerset County, New Jersey.
