Is HRT covered by insurance?
Many women assume menopause care means paying out of pocket, especially from online services. In most cases your insurance, including Medicaid, covers it.
The short answer: usually, yes. Menopause visits are billed like any other medical visit, and generic estradiol and micronized progesterone are on most plans' drug lists, including Medicaid and Medicare drug plans, often at a low copay. Brand-name products and the newer non-hormonal medicines may need prior authorization. Compounded hormones and pellets usually aren't covered.
At MyPremierMD we accept just about every insurance, including Medicaid, Medicare and commercial plans, for both office and video visits.

What prior authorization means
For some brand-name medicines, your plan wants a reason before it pays, often that you've tried a generic first or that a generic isn't suitable. We send that paperwork for you, and if it's denied, we don't simply give up. Our team files appeals, sometimes several rounds of them, to get medicines like Veozah approved when they're the right choice. If an appeal truly can't succeed, there's usually an effective generic alternative, and we'll talk through the options rather than leaving you with a prescription you can't fill.
Coverage details differ between plans, even within the same insurance company, so we check yours before we prescribe anything expensive.
We'll check your coverage before we call.

Ways to keep costs down
- Ask for generics. Generic estradiol and progesterone work as well as brand-name versions.
- Consider a 90-day supply once your dose is settled, which often lowers the copay.
- Choose the right form for your plan. If your plan prefers patches over gels, for example, we'll work with that when it's safe to. Patch vs. pill vs. cream
- Use video visits to save time off work and travel, billed the same as office visits. Menopause telehealth
- Skip unregulated add-ons like saliva tests and compounded creams, which you'd pay for yourself.
Why this matters
Cost shouldn't be the reason a woman goes without treatment for years of hot flashes, sleepless nights and low mood. Many online menopause services charge a membership or cash fee. As a local practice that bills just about every insurance, including Medicaid, we can offer the same modern treatments with the same clinician at every visit, and your plan does most of the paying. Insurance and cost questions
Is hormone replacement therapy covered by insurance?
In most cases, yes. Menopause visits are billed like any other medical visit, and generic estradiol and micronized progesterone are on most insurance drug lists, usually at a low copay.
Does Medicaid cover HRT in New Jersey?
Medicaid plans generally cover medically necessary menopause visits and common generic hormone therapy. Brand-name products may need prior authorization. We accept just about every insurance, including Medicaid, and check your specific coverage.
Does Medicare cover hormone therapy?
Medicare Part B covers the visits, and Part D drug plans usually cover generic estradiol and progesterone. Coverage for specific brand-name products varies by plan.
Are Veozah and Lynkuet covered by insurance?
Many plans cover them, often after prior authorization or after trying a generic option first. We handle the prior authorizations and keep pushing with appeals when a plan says no, and we can suggest effective generic alternatives if an appeal truly can't succeed.
Are compounded hormones or pellets covered?
Usually not. Compounded bioidentical hormones and hormone pellets are typically paid out of pocket. FDA-approved bioidentical estradiol and progesterone are covered by most plans and are what we generally recommend.
Are telehealth menopause visits covered?
Yes. Our video visits are billed to your insurance just like office visits, as long as you're in New Jersey at the time of the visit.
