Online Scheduling: Our online appointment scheduling system is provided as a convenience. We will confirm all appointments, but we cannot guarantee that your preferred time will be available.

Cancellation Policy: If you need to cancel or reschedule an appointment, please notify us at least 24 hours in advance. This allows us to offer the appointment time to other patients who need care.

No-Show Policy: Repeated missed appointments without notice may result in limitations on future scheduling or discharge from the practice. We understand that emergencies happen, so please contact us to discuss your situation.

Same-Day Appointments: While we strive to accommodate same-day appointments, availability cannot be guaranteed. Walk-ins are welcome during regular office hours subject to provider availability.

PremierMD offers telemedicine services for eligible patients and conditions. By using our telemedicine services, you acknowledge and agree that:

• Telemedicine has limitations and may not be appropriate for all conditions
• You are responsible for ensuring a private, secure location for your telemedicine visit
• You need a reliable internet connection and compatible device
• Technical difficulties may occur and could interrupt service
• Your provider may determine that an in-person visit is necessary
• The same privacy and confidentiality standards apply as in-person visits

Payment Responsibility: You are responsible for payment of all services rendered. Payment is due at the time of service unless other arrangements have been made in advance.

Insurance Verification: While we will verify your insurance coverage as a courtesy, it is ultimately your responsibility to ensure your insurance is active and covers the services you receive.

Non-Covered Services: If your insurance does not cover a service you receive, you will be responsible for the full cost. We will inform you when possible if a service may not be covered.

Jurisdiction: You agree that any legal action or proceeding arising out of or relating to these Terms or the Service shall be brought exclusively in the federal or state courts located in Somerset County, New Jersey.

Waiver of Class Actions: You agree that any dispute resolution proceedings will be conducted only on an individual basis and not in a class, consolidated, or representative action.
